This card will not be charged without notice and is there as a backup in case of failed/late payment or lack of insurance coverage. Every effort will be made to contact the patient before charging a stored card.
The treatment room for in-office appointments is located inside Anna's Hair Salon in the Canex building. Doors to the building are locked at 5pm, any appointments starting at that time or later will need to wait for me to open the doors. The hair salon operates Tues-Fri 10am-4pm, so there will be extra activity during these time slots. If you prefer a quieter environment, please look at booking Mondays.
Mobile appointments that are over 20km away from the business address will incur an additional 75¢ per km fee (round trip). This fee is not billable to insurance. Distance initially calculated by Google maps, and verified by odometer upon arrival to the appointment.
